As a registered dietitian-nutritionist member of the Ordre des Diététistes Nutritionnistes du Québec (ODNQ) and in accordance with the Code of Ethics for dietitians, as well as Law 25 concerning the protection of personal information, we are obligated and committed to respecting the confidentiality of information collected on our website and during the course of our duties as dietitian-nutritionists.

This Privacy Notice applies to personal information collected by NutriSkulpt, Intuitive Eating, Digestive Health, Sports Nutrition. Personal information is collected through the website, https://nutriskulpt.com/. Below, we provide you with an overview of our personal information handling practices, as well as how to contact us for any privacy questions or inquiries.

  1. Types of Information We Collect
    We collect personally identifiable information which may include your name, date of birth, e-mail address, contact phone number, mailing address, sex, and/or gender. For a more accurate list of the personal information collected, please refer to section 8 of the document.

    We collect your personal information from the following sources:

    1. Information you give us when you fill out online inquiries received through our e-mail form, client identification forms, and information sent via social media channels such as Instagram or Twitter. We may also collect personal information from any written correspondences, in person-meeting, phone consultations, video conferencing, and other forms of telecommunication;
    2. information from other sources including without limitation, third parties, counter parties, agents, trustees, advisors, consultants, representatives etc.; and
    3. information that we collect automatically when you visit our website, such as information about your browser settings and other information collected through cookies.
  3. How We Use and Disclose Your Information
    1. Email and names will be collected and used solely by NutriSkulpt. We use your information to provide services and to manage our business operations. For example, we use your personal information in order to fill your orders, answer your inquiries, enroll you in an online course, register to download e-books or analyze our business operations and website performance.
    2. We use your information for marketing and promotional purposes, including with our promotional partners if any.
    3. We combine all the information we collect other than information relating to your browsing history.
    4. We share information with our affiliates who may be outside of Canada and may be subject to laws that are different from those in Canada.
    5. We use third parties to assist us in our operations. We may share your information with these third parties in order to provide services to you.
    6. Please note that NutriSkulpt is not a custodian of health information. We do not retain personal health information provided by our customers.

    1. You may notify NutriSkulpt of your communication preferences with respect to e-mail newsletters and notifications of special offers and products by sending your communication to info@nutriskulpt.com.
    2. You may also close your account if you wish to withdraw your consent from our further use of your information. We may, however, retain information about your previous transactions for audit purposes, to ensure the integrity of our data and to fulfill legal requirements.

We are located at 3 Place Ville Marie Suite 400, Montreal, QC, Canada.NutriSkulpt ’s Privacy Officer is accountable for our personal information handling practices.     In this Privacy Notice, the terms “we”, “our” and “us” mean NutriSkulpt .

  3. What Personal Information Do We Collect?
    Personal information is information that identifies you or could be combined by us or our service providers and affiliates with other information to identify you. This information includes your personal e-mail address, your home address, your home telephone number, your personal cellphone number, your image, and may include your age, your anthropometric measurements, your medical information, your medication, your blood test results (if and only if you send them to your Registered Dietitian at NutriSkulpt) and other similar information when associated with you. Personal information may also be information containing details as to whether you have opened our e-mails or how you have used our website, if we can associate that personal information with you. If you are a business customer or prospective business customer, personal information does not include your title, business address and business telephone number when we use that information to contact you in your business capacity.We collect personal information from you when you complete our online e-mail form on our website, www.host2002.temp.domains/~tilfoodd/nutriskulpt.  We also collect personal information when you complete a client information sheet, sign a contract for NutriSkulpt consulting services, enter a contest or promotion, take a survey, or otherwise submit personal information to us with your communication. For example, we will request credit card and other identifying information to process payment over phone, e-mail, or on our website. We will also ask you for contact information in order to provide an invoice, or communications related to the delivery of products and or services to you or the recipient that you designate.We receive personal information about you from other sources. For example, we will receive information from credit card processors regarding whether the credit card details you gave us have been accepted or declined. We will receive information from shipping companies regarding whether the materials we shipped were delivered or was undeliverable. We may also acquire information about you from other promotional or marketing companies with whom you shared your information and whom you have allowed to provide information to us.

This includes limiting access to, and the use of, your personal information through the use of passwords and graduated levels of clearance. We do not publish all of our security measures online because this may reduce their effectiveness. We take physical precautions to ensure that the computer servers on which your personal information is stored and archived are secure and that access to such servers is protected.Although we take precautions against possible breaches of our security systems, no company can fully eliminate the risks of unauthorized access to your personal information and no website is completely secure.
